Free Initial Consultation

This is the starting point for all new clients. We'll discuss your needs and how we are best able to serve you and your family

Customized Assessments

Assess the client's medical, functional, cognitive, emotional, and social status to identify problems, make recommendations, and assist in implementing a plan of care

Individualized Care Planning

A comprehensive individualized care plan, based on client needs or family requests

End of Life Care

Provide compassionate guidance and support to clients and their families coping with end of life care and challenges

Counsel, educate, and assist with the emotional needs of clients and families

Provide Education

Provide insight to family members about diagnoses, medications, disease processes, care options, and other helpful information to guide them through the many aspects of the aging process

Communication

Communicate with loved ones and professionals regarding client's status, well-being, and changing needs

Family Issues

Mediate between family members’ difference of opinions

Nutrition

Assess and ensure client has adequate food in the house and arrange for assistance with grocery shopping and meal preparation

Relocation

Assist with relocation when a person is no longer able to live at home safely

Help locate a proper assisted living or nursing home and assist with the process, visiting regularly, monitoring to ensure quality care, and advocating for client rights

Medication Management

Review medications, assess the client’s ability to take medication as prescribed, and provide medication management

Home Care Service Oversight

Screen, arrange, and monitor in-home care services

Client Appointments

Coordinate transportation for and accompany clients to appointments

Community Resources

Identify and connect client to community resources to help keep loved ones safe at home

Wellness Visits

In-person visits at home or a facility to oversee client's care and overall status, serve as advocate and liaison, and report findings to family

Family and Caregiver Coaching

Help families and caregivers navigate the complex healthcare system, set priorities, and make informed decisions

Crisis

Assist client and family in crisis situations and ER visits

Medical Advocacy

Assist in finding doctors and specialists for clients

Establish rapport with doctors and specialists and keep apprised of recent changes, updates in medication, and information gathered at prior appointments

Attend plan of care meetings at nursing homes and assisted living facilities

Health Insurance

Review insurance coverage and educate about options to ensure client is getting the maximum benefits available

Provide clarification about Medicare and Medicaid

Assist with the application process and plan comparisons

Refer and connect to insurance specialists, if needed

Remind of open enrollment periods

Bookkeeping and Bill Paying

Provide identity theft awareness training, monitoring, and alert family of any scam risks

Assist clients with bill paying, paperwork, financial documentation, medical forms, advance directives, and assist with Medicaid planning and applications
